Looks like it changed in the 1998/99 season, where it went from midweek games every month September - April before that to the fewer windows with 2 games in in October 1998, but there was still 1 midweek game in Feb and April 1999.

The play offs England v Scotland in November 1999 for Euro 2000 being one of the first major Autumn 'windows'

Suggest the expansion of the Champions league and needing more midweek windows for that during the season was also a driving factor
